2007 PBME Patient Safety Award
Recognition for Dedication to Excellence in Patient Safety

The 2007 PBME Safety Awards were presented at a joint department meeting on January 9th. Awards are given to individual
department members or teams that best exemplify the patient safety culture that we strive to create. This can be identifying
and/or correcting a hazard, making a special effort to reduce risk, or identifying personal weaknesses or errors and teaching
others to develop ways to prevent them.
Two $500 awards were presented this year.
Ross Jacques (BWH) won the award for his "persistence in eliminating technology risks to
patients". Ryan Forde (MGH) was recognized for "continuous attention to the solution of
patient safety challenges". A special $200 award was presented to Ellen Kinnealey (MGH) for her
"frequent innovation in providing solutions to unique safety problems".

At MGH, Jeff Cooper presented Ryan Forde with the PBME Annual Patient Safety Award.
"Ryan works on an everyday basis to keep things safe in the OR.". . ."He is proactive and works to put systems
in place and not just band-aids to fix things, always thinking ahead."
. . ."When something does go wrong, he not only
fixes it, but he goes further to think about how to prevent it in the future."

Michael Fraai presented the award to Ross Jacques at BWH.
"We on the Biomed team feel that Ross should be recognized for the excellence he does and the care that he takes, most particularly when working on the anesthesia machines". . . "his meticulous obsession on SM's on anesthesia machines, that must have saved someone's life."

Jeff Cooper presented Ellen Kinnealey with a special safety award.
"A 16 month old child was to undergo a stem cell transplant. In the past our unit had used regular baby monitors to listen
for the child behind the double closed doors if the parents could not be there. With our new monitoring system this was not
possible. . . . Ellen developed a system using a speaker phone in the child's room and one at the desk. . . . It had a
wonderful outcome for the patient and family. . . . Ellen also did a wonderful job with the new pumps for pediatrics.
She and her staff were always available for problems and responded with many helpful hints."
